Несмотря на то что метотрексат является препаратом первой линии при лечении ревматоидного артрита, у значительной части пациентов наблюдаются первичная или вторичная резистентность к терапии, а также развитие нежелательных лекарственных реакций. Одной из причин вариабельности терапевтического ответа могут быть генетические и эпигенетические факторы, регулирующие транспорт препарата в клетку и активность провоспалительных сигнальных путей. Цель исследования – изучить роль инсерционно-делеционного полиморфизма rs946631560 гена SLC19A1, однонуклеотидного полиморфизма rs1800795 гена IL6 и длинной некодирующей РНК IL6-AS в формировании терапевтического ответа на метотрексат у больных ревматоидным артритом. Материалы и методы. Проведено генотипирование полиморфных локусов rs946631560 гена SLC19A1 и rs1800795 гена IL6 методом ПЦР и ПЦР-ПДРФ. Уровень экспрессии генов SLC19A1, IL6 и длинной некодирующей РНК IL6-AS определяли методом количественной ПЦР в режиме реального времени, содержание IL-6 в плазме крови – методом иммуноферментного анализа. Статистическая обработка включала анализ соответствия распределения генотипов равновесию Харди-Вайнберга, критерий χ², расчет отношения шансов (OR) с 95% доверительным интервалом, непараметрические методы сравнения и корреляционный анализ. Результаты. Установлено, что гомозиготный генотип I/I полиморфного локуса rs946631560 гена SLC19A1 ассоциирован с повышенным риском развития ревматоидного артрита (OR = 5,581; 95% ДИ 1,574–19,793) и сопровождается снижением экспрессии гена SLC19A1. Для полиморфизма rs1800795 гена IL6 выявлена ассоциация аллеля С с развитием заболевания (OR = 2,533; 95% ДИ 1,799–3,565). Показано статистически значимое влияние данного полиморфизма на экспрессию длинной некодирующей РНК IL6-AS (F = 4,83; p = 0,01). Выявлена тесная положительная корреляция между уровнями экспрессии IL6-AS и IL6 (r = 0,80; p &lt; 0,001), а также отрицательная корреляция между экспрессией IL6-AS и SLC19A1 (r = -0,60; p &lt; 0,001). Кроме того, установлено статистически значимое влияние уровня IL6 на экспрессию SLC19A1 (F = 22,97; p &lt; 0,001). Заключение. Полученные результаты свидетельствуют, что полиморфные варианты rs946631560 гена SLC19A1 и rs1800795 гена IL6 вовлечены в формирование предрасположенности к ревматоидному артриту и могут определять эффективность терапии метотрексатом.
