Social Dispatch Assistance with Iron Deficiency Anemia in Students with the Consequences of Early Organic Damage to the Central Nervous System

- The chapter is devoted to determining the prevalence of iron deficiency anemia (IDA) in students with the consequences of early organic damage to the central nervous system. The increased interest in this disease is due to a number of circumstances. Firstly, hypoxia of the brain in IDA affects the cognitive abilities of minors. Secondly, this disease in children with intellectual disabilities provokes diseases of the cardiovascular system in 62.5%, visual impairment in 31.2%, and pathology of the urinary system in 38.7%. Research methods. For the presence or absence of iron deficiency anemia, in the period from 2015 to 2023, medical documents of 653 students from different nosological groups associated with cognitive impairment were examined. The study made it possible to draw the following conclusions: the prevalence of IDA in children with F80 (expressive speech disorder) is 20%, with F83 (mixed specific disorders of psychological development) – 31%, with F70 (mild mental retardation) – 52%, with F71 (moderate mental retardation) – 33%. The total number of minors with a history of iron deficiency anemia is high. More than 40% of all students and pupils with disabilities (HIA) who study according to adapted programs due to intellectual and speech disorders may need social dispatching assistance from specialists of PMPC and school councils. The results of the study may be of interest to a wide range of medical and pedagogical specialists. Doctors are asked a problematic question about why, with moderate mental retardation and more pronounced (significant) cognitive impairments, the prevalence of IDA is significantly lower compared to mild mental retardation. Using specific clinical examples, teachers are described various algorithms of social dispatching assistance to families to prevent possible secondary violations in minors.

- Статья призывает специалистов психолого-медико-педагогических комиссий (ПМПК) и школьных консилиумов целенаправленно изучать наличие у детей с ранним органическим поражением центральной нервной системы кровотечения из носа, оказывать посильное содействие в устранении этого негативного явления (патологического процесса) и, тем самым, предупреждать возможные летальные случаи среди несовершеннолетних. Проведенное исследование по изучению распространенности кровотечений из носа среди дошкольников и школьников с ограниченными возможностями здоровья (ОВЗ) подчеркивает важность и необходимость этой социально-диспетчерской работы. Было изучено 293 клинических интервью с родителями детей, имеющих интеллектуальные нарушения (смешанные специфические расстройства психологического развития, умственную отсталость легкой и умеренной степени), которые являются следствием раннего органического поражения ЦНС. Проведенное исследование позволило сделать следующие выводы: 22,9% от общего числа всех воспитанников и обучающихся с ранним органическим поражением центральной нервной системы страдают кровотечениями из носа; в отдельных же нозологических группах частота кровотечений из носа у несовершеннолетних, которые не получают специального лечения, может достигать 77,3%. В работе представлен алгоритм направления ребенка на консультацию к детскому врачу-неврологу, в безотлагательной помощи которого усматривается острая необходимость. Результаты исследования могут заинтересовать широкий круг медицинских, а также педагогических специалистов, которые работают в современных условиях реализации Федеральных государственных образовательных стандартов (ФГОС) в системе центральных и территориальных ПМПК, школьных консилиумов.

Determining the Total Number of Endocrine Disorders Among Children With Speech Disorders: Social and Dispatching Work

- The article is devoted to determining the total number of endocrine disorders of the thyroid gland among children with speech disorders based on medical ultrasound data. The author of the article outlines that due to the fact that the prevalence of speech disorders is 100% among children with hypothyroidism, the following question deserves attention: “How many endocrine disorders can occur in children with speech disorders?”. Methods of research. Within the framework of the social and dispatching work of specialists of the psychological-medical-pedagogical commission, the referral of children with speech disorders to a medical ultrasound examination of the thyroid gland was organized. At the same time, none of the children had previously been examined by an endocrinologist or received appropriate treatment. The first group of children attended speech therapy classes at the Center for Psychological and Pedagogical Rehabilitation and Correction in Nizhneudinsk in the 2013-2014 academic year. The second group of children attended preschool educational institution №208 in the 2016-2017 academic year. The study allowed to draw the following key conclusions: in children with speech disorders in 40% of cases do have previously undetected endocrine disorders that deserve close attention and study by medical professionals, both of primary health care institution (Institute of Pediatrics) and pediatric endocrinologists; 29% of children with speech disorders potentially need the help of an endocrinologist. The algorithm for sending parents of children with speech disorders to consult medical specialists, whose help the child may need is presented in the article. The results of the study may be of interest to a wide range of medical and pedagogical specialists working in the system of school psychological and pedagogical councils, central and territorial psychological and medical and pedagogical commissions in the current conditions of the Federal State Educational Standards (FSES) implementation.
